RESUMO

Very often clinicians are confronted with unconscious patients; the cause being hypoglycaemia. In addition to correction of hypoglycaemia promptly, a search for the cause of hypoglycaemia must be attempted at the earliest with a stepwise approach. Here, the authors present a rare case of recurrent hypoglycaemia where a stepwise approach ultimately led to a diagnosis, but there were various reports which were misleading. The diagnosis was insulinoma which was apparent from the history and presenting features. But, the initial investigations, even though having high sensitivity, were normal, thus leading to a more invasive approach. The patient was ultimately cured by enucleation.

Background: To study the lipid profile of 3rd trimester pregnant women and their cord blood with and without intrauterine growth restriction Design: Observational study, Setting: Department of Obstetrics and Neonatology, in a teaching hospital in North India during February 2013 to August 2014.Methods: Third trimester pregnant women and their neonates. Enrolled 250 women were divided in intrauterine growth restricted and control groups. Outcome Measures: Venous blood Lipid levels of 3rd trimester mothers and their neonate at birth.Results: Women of IUGR group had significantly lowered total cholesterol (TC), triglycerides (TG), high density lipoproteins (HDL) and low density lipoproteins (LDL)210.2 (19.8), 221.6 (36.2),65.4(11.7)and 130.7 (20.8)compared to 251(55.3), 234.7(35.6), 70.8(19.9) and 181.3(16.8) mg/dl control group. The TC, TG and LDL levels in cord blood of IUGR group were 93.6(16.5), 50.4 (6.5) and 51.8(12.6) as compared 106.4(17.7), 30.7(4.4) and 55.9(12.1) mg/dl to control group (p value <0.05). HDL levels were significantly lower 15.8(4.6) in IUGR group as compared to 26.5(5.4) in control group. The LDL: HDL and TC: HDL ratio was significantly higher in IUGR group. Maternal weight, gestational age, urban residence, primi parity, birth weight and APGAR score were lower, while maternal age, parity, smoking, blood pressure, cesarean sections (%) and male sex (%) of baby was higher in IUGR group.Conclusions: Lipid profile of mothers of IUGR fetuses had significantly lowered cholesterol levels and their cord blood had shown atherogenic phenotype

The present study was conducted to develop chicken meat powder (CMP) incorporated ready-to-eat shelf stable fried chicken snacks and evaluate the effect of rosemary leaves extract (RE) incorporation on physico-chemical, microbiological and sensory properties in developed product during ambient storage up to 60 days. Two different groups were made; control (without RE) and second group with RE treated (3% level). In physico-chemical properties, results showed that RE incorporation had highly significant (p<0.01) effect on thiobarbituric acid reactive substances (TBARs), free fatty acid (FFA) and tyrosine value. Similarly in microbiological parameter RE treated product had significantly (p<0.05) lower total plate count (TPC), Staphylococcus count (SC) and significantly (p<0.01) lower yeast and mold count than control. RE incorporation had highly significant effect (p<0.01) on sensory score (texture, flavour and overall acceptability except appearance) of the product during storage period. Therefore, it is concluded that RE incorporation into fried chicken snacks improved physico-chemical (TBARs, free fatty acid content and Tyrosine value), microbiological (Total plate count, Staphylococcus count and yeast and mold count) and sensory parameter (flavor, texture and overall acceptability) of the chicken snacks during 60 days storag

Background: The recently identified hormone kisspeptin has been suggested to play an important regulatory role in placentation. The aim and objective of the study is the measurement of serum kisspeptin level in asymptomatic pregnant women and to find out the association of serum kisspeptin with gestational age in women with early pregnancy.Methods: This was a longitudinal study to the evaluation of 178 asymptomatic pregnant women with a gestation of 6 to 16 weeks attending routine antenatal booking visit recruited as study participants from the Antenatal Clinical of Obstetrics and Gynaecology Department, S.M.S. Medical College and Attached Hospitals, Jaipur, Rajasthan, India.Results: After initial clinical examination of every participant, a single blood sample was taken for the measurement of serum kisspeptin. Serum kisspeptin measurement test was performed by ELISA method and results were expressed as ng/ml. Pregnancy outcome was recorded prospectively. Mean serum kisspeptin level of study participants was 2.80±1.87ng/ml and median were 2.41 (Range 0.244-14.06ng/ml). Our result showed the relationship of serum kisspeptin with gestational age (GA) (p<0.000).Conclusions: serum kisspeptin level increases in pregnancy and showed positive relationship with gestational age significantly (p<0.000).

Introduction: Spondyloarthropathies are a heterogeneousgroup of inflammatory interrelated diseases involvingperipheral joints and spine. All forms of spondyloarthropathiesare known to have a strong association with HLA-B27 gene invarious studies done. There was a paucity of literature aboutclinical and demographic characteristics of the HLA-B27positive arthritis patients in Rajasthan. So this study wasplanned to know the seropositivity of HLA-B27 amongseronegative spondyloarthritis patients and to find outassociation between socio-demographic and clinicalcharacteristics and HLA-B27 seroprevalence.Materials and Methods: A cross sectional observational studywas carried out from May 2017 to April 2018 in Department ofPathology, SMS Medical College, Jaipur (Rajasthan). A total of100 cases of spondyloarthritis disease were enrolled in thestudy to find out seroprevalence of HLA-B27 and todifferentiate the clinical and demographic characteristics inHLA-B27 positive and negative patients.Results: In the present study 29% seroprevalence of HLA-B27was observed. Seropositivity of HLA-B27 was significantlyassociated with age (P-value < 0.05) while sex, religion, caste,family history, raised ESR, Positive C-reactive protein, RAfactor etc. were showed no significant association (P-value >0.05).Conclusion: This study confirms the previously reportedassociation in ankylosing spondylitis patients between HLAB27 and earlier disease onset with male preponderance &family aggregation.
